1. All of your medical insurance cards, both primary and secondary
2. Your co-payment (cash or check; we do not accept credit cards)
3. For Keystone and Aetna HMO patients: you must have your primary care physician issue a referral into the appropriate computer system; hand-written referrals are no longer valid.
4. A complete list of your medications
5. Any pertinent medical records, biopsy reports, and X-rays
6. The name and address of your referring physician
